Two men with the same regimental number, same Bn.

I recently came across two Worcs. 2nd Bn. men with the same serial number 9401. Alfred Hymas and Frank George Blinko.
My source is the record from the findmypast British Army Service Records 1914-1920 . Frank G Blinkos medal Card and SWB record.

Frank Blinko was born 1883 in Hatfield and there are both medal index card and Silver war badge records for him with the serial number 9401 and specified as served with 2nd Bn. Worcs. Frank enlisted 23 Aug 1905 according the SWB record.
Of course the medal card and the silver war badge record his last Bn and regiment and so its possible he was with another Bn. until after Alfred Hymas was KIA Sep. 26, 1915 .

Anyone seen this kind of error before or can offer an alternative theory to how this could have happened?
